24/25 Statement of Identity & Educational Purpose Verification

The Department of Education recently selected a large population of students who attended the 24/25 academic year to submit a Statement of Identity & Educational Purpose Verification Form

If you were selected for this verification process, please refer to page 2 of the form for instructions on how to submit the form to our office. We advise that students take action immediately to avoid adjustments or cancellation of their 24/25 financial aid. 

25/26 Upload Documents and Submit Appeals via the Bruin Financial Aid Portal

UCLA students (continuing and new admits who submitted their Student Intent to Register) will upload required documentation or appeals for the 25/26 academic year using the Bruin Financial Aid Portal.

If you received a request to submit a Statement of Identity for the 25/26 academic year, please refer to page 2 of the form for instructions on how to submit the form to our office. For compliance reasons, this form cannot be emailed or uploaded via the Bruin Financial Aid Portal.

 

25/26 Newly Admitted Students (Appeal Forms)

If you are a newly admitted student who has not submitted your Statement of Intent to Register and want to submit an appeal with Financial Aid and Scholarships, please submit your appeal and supporting documentation via documentation via the MyUCLA Message Center under the “Financial Aid: Newly Admitted- Submit Documents” queue.
